About this Webinar:
Ensuring safety within the home is a cornerstone of community health and patient safety. For residents of public housing, daily exposure to hazards ranging from fall risks and poor lighting to medication mismanagement and chemical exposures can exacerbate chronic disease, functional decline, and preventable injuries.
This webinar, led by Dr. Kevin Lombardi MD, MPH and Olajumoke Oladipo MPH of the NCHPH Department of Data Analysis and Evaluation, will provide an in-depth overview of practical strategies health centers can implement to identify and mitigate home safety risks.
Drawing upon epidemiological data, case studies, and promising practices from health centers across the nation, participants will learn how to operationalize home hazard assessments as part of comprehensive primary care and upstream drivers of health initiatives. The session will also introduce workflow models used by CHWs and nurses during home visits.
Through this session, clinicians, nurses, and CHWs will also explore how home-based assessments, home environmental screening, and coordinated follow-up can substantially improve safety outcomes and reduce emergency-department utilization among high-risk patients, especially older adults, persons with disabilities, and long-term public-housing residents.
